Please review and complete the Dance Guest Pass below. Once completed, turn the form in and a copy of your guest's ID to the front office (Building 4000). This must be turned in by Wednesday, April 29th at 2:30pm. Late submissions will not be accepted. Please allow 24 hours for review and approval.
