Please be sure to bring a printed copy of the following:

1.  Valid Arizona Driver’s License (no permits),  
2. Current Insurance Card For Vehicle - Valid For 3 Months  
3. Vehicle Registration 
4. Payment of $60 - Accepted Payments; Debit/Credit Card, Tap Pay, Check or Cash 
5. Completed Application!
